29 CFR 1910.1200(h)(3)(i): The employer did not provide employee training that included methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area (such as monitoring conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released, etc.): (a) 3502 River Point Parkway, Sheridan, CO 80110: On or about April 20, 2024, and at times prior, due to lack of recognition of a newly introduced hazardous chemical to the workplace, employees retrieving food product from the walk-in freezer, which contained 200 pounds of dry ice, were exposed to levels of carbon dioxide that exceeded the concentration Immediately Dangerous to Life or Health (IDLH) of 40,000 ppm as set by the National Institute for Occupational Safety and Health (NIOSH). This resulted in an employee being overcome by the hazardous atmosphere and losing consciousness. Employees were not trained on the methods and observations that may be used to detect the presence or release of a carbon dioxide.
