Manufactured Housing Enterprises, Inc., at a worksite located at 09302 US HWY 6, Bryan, Ohio 43506: On or about May 1, 2024, in the partitions area, the employer did not ensure The Original Radial Arm Saw returned to the starting position after cutting 2x6 boards for wall panels there by exposing employees to an amputation hazard. b. Manufactured Housing Enterprises, Inc., at a worksite located at 09302 US Hwy 6, Bryan, Ohio 43506: On or about May 1, 2024, in the Saw shop, the employer did not ensure The Delta radial arm saw returned to the starting position after cutting boards for specialty parts there by exposing employees to an amputation hazard.
