29 CFR 1910.178(a)(5): The user did not request, for trucks equipped with front-end attachments other than factory installed attachments, that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. On or about June 27, 2023, a Morse heavy-duty forklift-carrier attachment was being used to move metal drums around the ink production area without being marked to identify the attachment and adjusted capacity and limitations, exposing employees to struck by hazards.
