29 CFR 1926.1431(e)(1): The personnel platform and attachment/suspension system used for hoisting personnel was not designed by a qualified person familiar with structural design: (a)Henry Carlson Construction, LLC at 1508 West 22nd Street, Sioux Falls, SD 57105: On or about and at times prior to May 25, 2023, the controlling employer did not ensure the Doka working platform was designed by a qualified person for the purpose of hoisting personnel. This condition exposed employees and subcontracted employees to fall, crushing, and struck-by hazards.
