29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed. Employees engaged in manufacturing operations at 210 North 13th Street, Seward, Nebraska 68434, were exposed to electrical hazards in that an energized 3 phase 240 volt conductor had an unused opening on the side of the box that was not closed.
