This documentation was not available to those authorized to design, install, inspect, maintain, or operate electric equipment at the location: (a)Forest Products Distributors, Inc. at 5406 Old Folsom Road, Rapid City, SD 57703: On or about and at times prior to May 12, 2022, the employer did not ensure that all areas designated as hazardous (classified) locations under the Class and Zone system and areas designated under the Class and Division system established after August 13, 2007 were properly documented This condition exposed employees to hazards associated with fire and explosion hazards.
