29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a. 200 S. First St., Elizabeth, NJ The employer did not ensure that authorized employees were provided recertification training for powered industrial equipment at least every three years. Employees engaged in the recycling of scrap metal operated forklifts that included, but were not limited to Clark GCS15 and Komatsu, FG30HT propane forklifts, and were not evaluated and certified at least every three years.
