29 CFR 1910.178(l)(6): The employer did not certified that each powered industrial truck operator has been trained and evaluated as required by paragraph (l) of 1910.178; The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person performing the training and evaluation. (a) On and before 3/2/2022, in the yard where employees operate fork trucks. The employer did not certify the powered industrial truck operators were trained and evaluated as required by paragraph (l) of 1910.178. 1KB & MS LLC DBA Kingston Block & Masonry LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.146178(l)(6), which was contained in OSHA inspection number 1234830 citation number 1, item number 6b and was affirmed as a final order on 06/14/2019, with respect to a workplace located at 1 Kiefer Lane Kingston, NY 12401.
