29 CFR 1910.146(j)(1): The employer did not ensure that each entry supervisor knew the hazards that could be faced during entry, including information on the mode, signs or symptoms, and consequences of the exposure: Employees were exposed to chemicals hazards. Employees vacuuming lime pebble and granular quicklime from the L. G. Everist's lime pit located at 8440 Gibbs Rd. Kansas City, KS 6611 were exposed to dermal and inhalation hazards. The entry supervisor had not been trained and was unaware of the hazards associated with exposure to lime pebble and granular quicklime.
