29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: A) Morral Companies, LLC, located at 132 Postle Avenue, Morral, OH 43337: On or about April 16, 2020, the employer did not provide employees, performing service and maintenance on equipment with hazardous energy sources, with adequate lockout/tagout training. Employees were required to perform work in the Lawn Mixer, located in the Liquid Department, without adequate training in the recognition of the hazardous energy posed by the mixer's agitator, the type and magnitude of the energy powering the agitator, and the methods and means necessary for energy isolation and control.
