29 CFR 1926.1204(d): The following equipment (specified in paragraphs (d)(1) through (9) of this section) was not provided at no cost to each employee, maintain that equipment properly, and ensure that each employee uses that equipment properly: a. On or about December 29, 2018, at the above addressed jobsite the employer did not provide equipment necessary for entry into a confined space, thereby exposing employees to the hazards associated with confined spaces. Equipment required but not provided includes, but was not limited to ventilating equipment, testing and monitoring equipment, personal protective equipment, and lighting equipment.
