29 CFR 1910.119(f)(3): Operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice. On or around May 19, 2018 and at times prior thereto, at the Kuraray EVAL manufacturing facility in Pasadena, Texas the employer did not ensure written operating procedures were current and accurately reflected instructions for starting up the 1201 Reactor in the 1200 Polymerization process unit following a turnaround.
