(a) VLR Tanks - effluent trough area: On or about November 11, 2017, and times prior, outer insulation was damaged on a 75 foot,12AWG extension cord, used to power tools and equipment such as Dewalt angle grinders and a Hilti chip hammer. (b) Tank 3. On or about November 11, 2017, and times prior, outer insulation was severely worn and damaged on an unidentifiable orange extension cord, used during work in the effluent tank.
