29 CFR 1910.1052(i)(2): It was reasonably foreseeable that an employee's eyes may contact solutions containing 0.1 percent or greater methylene chloride and the employer did not provide appropriate eyewash facilities within the immediate work area for emergency use and did not ensure that affected employees use those facilities when necessary: (a) On and before 02/08/2017, throughout spray area, for the employee wearing a dust mask while spraying duct work with adhesive containing ingredients such as, but not limited to, methylene chloride. Employee was exposed to methylene chloride at a short term exposure limit (STEL) of 178.740 ppm which has a severity of approximately 1.43 times the short term exposure limit (STEL) of 125.000 ppm. The sample was taken over a 15-minute period. The employer did not ensure the eyewash was filled with water for the employees to use in the event of eye contact with 55 percent methylene chloride.
