29 CFR 1910.146(d)(3): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to, items (3)(i) through (3)(vi) of this paragraph: a) 9000 River Rd, Delair, NJ, 08110- Anodizing: The employer did not develop and implement the means, procedures and practices necessary for safe entry into anodizing tanks including, but not limited to, acceptable air quality parameters, procedures to eliminate drowning hazards, to eliminate falling hazards, to control electrical energy in tanks with cathodes, determine the frequency of air monitoring when in the tank to determine acceptable atmospheric conditions and for the use of ventilation to control atmospheric hazards within the tanks prior to and during entry, on or about 2/3/2017. b) 9000 River Rd, Delair, NJ, 08110- Foundry: The employer did not develop and implement the means, procedures and practices necessary for safe entry into the furnace pits including, but not limited to, acceptable air quality parameters, procedures to eliminate fall hazards, visibility hazards, struck by and crushing hazards, heat stress, for barricading the space from forklift and pedestrian traffic, for ventilating the space prior to and during entry and to verify air quality before and throughout the entry, on or about 2/14/2017. c) 9000 River Rd, Delair, NJ, 08110- Foundry: The employer did not develop and implement the means, procedures and practices necessary for safe entry into the casting pits including, but not limited to, acceptable air quality parameters, procedures to eliminate fall hazards, visibility hazards, drowning hazards and crushing hazards, for barricading the space from forklift and pedestrian traffic, for ventilating the space prior to and during entry and to verify air quality before and throughout the entry, on or about 2/14/2017. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
