29 CFR 1926.850(i): All floor openings, not used as material drops, shall be covered over with material substantial enough to support the weight of any load which may be imposed. Such material shall be properly secured to prevent its accidental movement. a. On or about August 4, 2016, located on the 3rd floor walkway to the southern end of the former Columbus Municipal Light Plant, there was a floor hole that was 5 feet, 10 inches long and 2 feet, 9-1/2 inches wide that had a piece of plywood over the hole. The plywood was not secure, thereby exposing the employees to a fall hazard of approximately 21 feet. b. On or about August 4, 2016, located on the 5th floor of the former Columbus Municipal Light Plant, there were two 17 inch floor holes with unsecured covers, thereby exposing the employees to a trip/fall hazard.
