29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 m) above the walking/working surface, were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system. Location: 520 Belleville Ave, Belleville, NJ. a) Employees were exposed to falling to the ground through inadequately guarded window openings that had no toprails and the sills were only 31.5 inches high and the height and width of the window openings were approximately 80 inches and 40 inches respectively. The employees were exposed to potential falls ranging from 20 ft. to 50 ft. to the ground below; on or about 2/22/16. b) Employees were not protected from falling while walking/working near elevator shafts that were not guarded and others shafts had a plywood board (not secured) with gaps in excess of 18 inches on the sides. Shafts had a height of approximately 10 ft. and a width of approximately 5ft. The employees were exposed to potential falls ranging from 20 ft. to 70 ft. to the ground below; on or about 2/22/16.
