29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) Sign Source USA, Inc. - Lima, Ohio: On or about December 15, 2015, the employer failed to train an employee painting in the small paint booth on the hazards of the chemicals in the work area. Employees were exposed to hazards from various organic vapors and isocyanates such as, but not limited to, n-butyl acetate, hexone, xylene, methyl ethyl ketone, and 1,6 - hexamethylene diisocyanate homopolymer. Sign Source USA, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection #110898, Citation 01, Item 8, Group (b) and was affirmed as final order on April 25, 2012, with respect to a workplace located at 1700 S. Dixie Hwy, Lima, Ohio 45804.
