29 CFR 1910.212(a)(5): Fan blade guard(s) were not provided where the periphery of the blades was than seven (7) feet above the floor or working level, the fan's blades were not guarded by a guard which had openings no larger than one-half (1/2) inch. (a) Maintenance Shop: On or about June 25, 2015, and times prior, employees use and work around an industrial floor fan measuring 75 inches wide by 60 inches in height that was not provided with a guard on the backside to cover the four 20 inch rotating blades.
