29 CFR 1910.146(e)(6): The employer did not retain each canceled entry permit for at least one year to facilitate the review of the permit-required confined space program required by 29 CFR 1910.146(d)(14): Jobsite located at 158 Cronin Avenue, Forestville, Connecticut: On, or about, June 15, 2015, the employer assigned a crew to complete repairs inside of two circuit breaker tank enclosures. The permit(s) issued for entry into the confined space was not retained for at least one year following the completion of the job.
