29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about October 31, 2014, and at times prior thereto, the distance between the adjustable guard exceeded one-fourth inch exposing employees to struck-by hazards: a) Between rows-E and F, the DEWALT Model DW 756, 6-inch Bench Grinder, Serial No. 2009 51-YL37941 was set at an approximately seven-eighths of an inch opening from the wheel. b) Inside the Maintenance Tool Room, the 1/2 HP Baldor Bench Grinder Cat No. 712, Serial No. W584, opening from the wheel was approximately 1-1/8 inches on the left and 1-inch on the right.
