Text For Citation: 01 Item/Group: 006A Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that caused or were likely to cause death or serious physical harm in that employees were exposed to fall hazards: Loading dock at 221 W. 38th St On or about September 18, 2014 a) An employee was working from an elevated Snorkel platform lift that was positioned on a sloped sidewalk, exposing that employee to a fall hazard of 25 feet. b) An employee was working from an elevated Snorkel platform lift and did not wear a safety belt. c) An employee was in the elevated platform while another employee moved the elevated employee. d) Maintenance and annual inspections on the Snorkel platform lift were not completed and documented. Among other methods, one feasible and acceptable method to correct this hazard is to comply with the manufacturer's (Traveler's) recommendation and ANSI/SIA A92.3-1990 sections 7 & 8: The "Traveler" is to be elevated ONLY on firm, level surfaces." All Personnel in platform MUST wear a safety belt. Secure safety belt to work platform, NEVER to an adjacent structure. The Traveler is a one man elevated work platform which must have outrigger set before the platform is elevated. Maintenance and annual inspections were not completed and documented as required by the Manufacturer's operation and Maintenance Manual and ASNI/SIA 92.3-1990.
