29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): Employees were exposed to falls, lacerations, cuts and impalement hazards when adjusting and clearing material on the Old Shear sorting house conveyor without operating energy isolating devices to control hazardous energy sources. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
