MCM Precision Castings, Inc. located in Weston, Ohio: On or about July 17, 2014, the employer did not ensure employees were wearing hard hats, flame retardant clothing, and spats when exposed to molten metal splashes and sparks when receiving molten metal from the induction furnace to heat the transfer ladle. c.
