29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complied with recognized and generally accepted good engineering practices (RAGAGEP). a)Engine room: On or about October 4, 2010 and at times prior thereto, the employer did not document that the air intakes complied with RAGAGEP in that, they were not equipped with operable means of control to prevent reverse flow. b)Engine room: On or about October 4, 2010 and at times prior thereto, the employer did not document that the roof air intake complied with RAGAGEP in that, it was not arranged to prevent short circuiting with the adjacent exhaust fans. c)Engine room: On or about October 4, 2010 and times prior thereto, the employer did not document that pipes penetrating the ceiling and interior walls complied with RAGAGEP in that, they were not tightly sealed with the ceiling or wall. d)Engine room and roof: On or about October 4, 2010 and times prior thereto, the employer did not document that a relief header where two or more relief devices were connected complied with RAGAGEP in that, it was not properly designed to accommodate the worst case scenario. e)Engine room: On or about October 4, 2010 and at times prior thereto, the employer did not document that the ventilation system complied with RAGAGEP in that, it was not equipped with a mechanical failure alarm. f)Roof near condensers: On or about October 4, 2010 and at times prior thereto, the employer did not document that the relief vents complied with RAGAGEP in that, they were not positioned at least 15 feet above the walking surface. Abatement Note: Among other methods to comply with recognized and generally acceptable good engineering practices the employer may consider the following: ensure that air intakes are equipped with a means to prevent reverse flow per 1994 Uniform Mechanical Code; ensure that air intakes are arranged to prevent short circuiting per 1994 Uniform Mechanical Code; ensure that pipe penetrations are tightly sealed with the ceiling or wall per 1994 Uniform Mechanical Code; design the relief headers in accordance with 1992 ANSI/IIAR-2; ensure ventilation system is designed in accordance with 1992 ANSI/IIAR-2; ensure that relief vents are located in accordance with 1994 Uniform Mechanical Code. Note: Abatement certification and supporting documentation are required for this item.
