Section 5(a)(1) of teh Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to potential hazards of electrical explosion resulting from employees working with and adjacent to emergized electrical cables in a padmounted transformer: (a) Jobsite, Adjacent to American Mathematical Society, 201 Charles Street, Providence RI: Two employees involved in the switching operation on energized electrical cables in a padmounted transformer were not adequately protected from the hazards of electrical explosions. One feasible method to protect employees doing this type of work is the following: 1. Conduct phase to phase testing to identify the phase configuration; %% 2. Provide employees performing the work a map of the circuit so that existing conditions can be determined before starting work; 3. Evaluate condition of transformer and clear up any congestion within the pad-mounted transformer; 4. Install permanent phase marking on transformer; 5. Conduct a job briefing before each job. This would include task sequence, determination of what tools are proper for job, what procedures should be used, clearance requirements, requirements for personal protective equipment, and special precautions such as phase configuration and other hazards which are unique to each job.
