29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service. (a) 64 Hess Road Leola, PA 17540 - Employees were exposed to electrical hazards while conducting modular building operations due to inadequately repaired yellow flexible cord in Plant 1, located on the floor, in a room of the modular building that was under construction, on or about June 6, 2024. (b) 64 Hess Road Leola, PA 17540 - Employees were exposed to electrical hazards while conducting modular building operations due to a yellow flexible cord missing its grounding pin in Plant 1, located on the floor, in a room of the modular building that was under construction, on or about June 6, 2024. (c) 64 Hess Road Leola, PA 17540 - Employees were exposed to electrical hazards while conducting modular building operations due to inadequately repaired purple flexible cords in Plant 1, located on the floor, in a room of the modular building that was under construction, on or about June 6, 2024. (d) 64 Hess Road Leola, PA 17540 - Employees were exposed to electrical hazards while conducting modular building operations due to a damaged flex cord on the floor fan in Plant 1, located on the floor, in a room of the modular building that was under construction, on or about June 6, 2024. (e) Employees were exposed to electrical hazards while conducting modular building operations due to inadequately repaired yellow flexible cord in Plant 1, located on the floor near the chop saw, on or about June 6, 2024. Abatement certification required within 15 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
