29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder. Jobsite: On or about December 12, 2023, the employer did not ensure that employees using 28-foot aluminum extension ladders at a height of approximately 21 feet, 8 inches from ground level to the roof deck, set the bases of the ladders approximately one-quarter of the working length away from the building.
