Text For Citation: 01 Item/Group: 006A Hazard:
29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Imperial Bag & Paper Co. LLC located at 350 4th Street, Perrysburg, Ohio: On or about September 19, 2023, the employer failed to ensure an energy control program consisting of control procedures, employee training and periodic inspections were developed and performed. Employees tasked with performing service and maintenance tasks such as, but not limited to, changing blades of the Harris Seybold Saber IV Power Back Gauge Cutler Hammer (Paper Cutter) without the benefit of energy isolation, hazardous energy recognition training and employer evaluation, exposed employees to amputation hazards created by the unexpected energization of the equipment.
