29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: a) On or about August 22, 2023, at the above addressed jobsite, the employer equipped a Caterpillar Telehandler TL1255D SN. ML700498 with a Material Handling Box that was not supplied or approved by the manufacturer to elevate personnel, thereby exposing employees to hazards including, but not limited to, struckby and caught between.
