29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment. a) 1204 E. 12th St., Wilmington, DE 19802 (Building #3 Panel # 3): On or about August 3, 2023, and at times prior thereto and thereafter, employees were exposed to hazards when the Western Electric Panel # Z 169510 were observed energized with missing parts and were not effectively closed exposing employees to shock hazards. b) 1204 E. 12th St., Wilmington, DE 19802 (Building #3 Panel # 4): On or about August 3, 2023, and at times prior thereto and thereafter, employees were exposed to hazards when electrical circuit panel boxes were observed energized with missing parts and were not effectively closed exposing employees to shock hazards. c) 1204 E. 12th St., Wilmington, DE 19802 (Building #4 General Electric Panel #7): Employees were required to enter an ineffectively guarded General Electric panel number Y-724632 to activate/deactivate overhead lighting daily. The electrical panel observed by CSHO displayed missing screws holes. The opened holes exposed employees to electrical shock hazards on a daily basis. d) 1204 E. 12th St., Wilmington, DE 19802 (Building #3/Rear Wall Trough): Employees were exposed to electrical wiring that was not effectively closed. e) 1204 E. 12th St., Wilmington, DE 19802 (Building #3/Rear Overhead Door Control): Employees were exposed to electrical wiring that was not effectively closed. f) 1204 E. 12th St., Wilmington, DE 19802 (Building #/4/unknown conduit picture number 20230803_174406134): Employees were exposed to electrical wiring that was not effectively closed. Four wires were observed coming from the conduit with wire nuts attached. g) 1204 E. 12th St., Wilmington, DE 19802 (Building #/4/unknown junction box picture number 20230803_174409474): Employees were exposed to electrical wiring that was not effectively closed. Three wires identified as green, red, black were exposed without a proper cover. h) 1204 E. 12th St., Wilmington, DE 19802 (Building #/3/unknown Light switch picture number 20230803_163535096): Employees were exposed to electrical wiring that was not effectively closed. CSHO observed a thermostat connected to a electrical switch that was not effectively closed.
