CG/ICP, was being added to windshield washer fluid from an open bucket, and suitable facilities for quick drenching or flushing of the eyes and body were not provided within the methanol manifold work area for immediate emergency use. The nearest eyewash was approximately 30-seconds away from the methanol manifold area where the corrosive material is used.
