29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a. 200 S. First St., Elizabeth, NJ The employer did not ensure that each employee authorized to operate powered industrial equipment was provided training and evaluations. An employee engaged in metal recycling activities operated a Komatsu, FG30HT forklift and was not provided training that included instruction, a test, and an evaluation and certification prior to using the equipment.
