Durable Corporation on or about October 13, 2021, at the address of 75 N. Pleasant St. N. Norwalk, OH: The employer did not ensure the foot operated pedal mechanism that cycles the press was guarded from accidental engagement on the Alva Allen Industries model BT12 12 ton full revolution press. In this condition employees are exposed to amputation hazards.
