29 CFR 1910.178(l)(4)(ii)(A): Refresher training in relevant topics shall be provided to the operator when the operator has been observed to operate the vehicle in an unsafe manner. On or about January 21, 2021, the employer did not conduct refresher training for PIV operators who were observed to operate the vehicle in an unsafe manner. (a) On 1/8/2021, an employee was coached, instead of receiving refresher training, for not using the horn of their PIV at intersections and ends of aisles in the warehouse. (b) On 12/22/2020, an employee was coached, instead of receiving refresher training, for not stopping their PIV and using the horn at the end of aisles in the warehouse.
