29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets. On or about November 11, 2020, an explosion occurred during the manufacturing of PureVista a water-activated product used to generate chlorine dioxide and the employer did not ensure that employees and management officials were provided effective training on hazards to which they were exposed. Effective training lacked one or more of the following elements: the combustible dust hazard and characteristics, the combustible nature of the dust, the ability to catch fire, deflagrate, and explode. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
