29 CFR 1926.1204(c)(3): The entry employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to: Isolating the permit space and physical hazard(s) within the space: a) On June 29, 2020, at the above address, the employer did not ensure the pit of the freight elevator system was isolated in accordance with its requirements from overhead hazards during overhead re-cabling operations. i. Overhead materials or equipment from outside of the overhead hoist way opening on the 6th floor. During re-cabling operations, two reels of hoist cable were suspended on a reel stand (holding two Greenlee Model 687 supports and a horizontal spindle of unknown origin). This equipment was located in the 6th floor hallway just outside of a hoist way opening of approximately 12.5" (between the bottom of the elevator car and the floor of the hallway), which was not adequately protected by a fixed toe board or other guard. The horizontal spindle was not approved by the manufacturer for that use and was therefore subject to falling from the reel stand (as recognized by the manufacturer). ii. Overhead tools located on top of the elevator car. During re-cabling operations, an unsecured 15/16" deep well socket wrench, with a ?" ratchet wrench was present on top of the elevator car. iii. Overhead hoist cabling. During re-cabling operations, hoist cabling was lowered down the hoist way and into the elevator pit.
