29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 3/21/2018, Dry Storage Building: Sheer point between a feed table and conveyor was not guarded. b) On or about 3/21/2018, Optimizer Area: Interlocks on Optimizer guards were made inoperable, which left feeder rolls not fully protected. c) On or about 3/21/2018, Mixer Area: The Black brand mixer used to mix glue was not provided with a guard. d) On or about 3/21/2018, Turning Area: A machine used to press bases on pins was not provided with guarding or two hand controls. e) On or about 5/21/2018, Rework Area: A rerun lather was in use without a guard or closed door.
