29 CFR 1910.25(c)(2): The employer did not ensure standard stairs have a maximum riser height of 9.5 inches (24 cm). On or about March 15, 2018, employees were exposed to slip and fall hazards when the riser heights were 16, 11, 11, and 10 1/2 inches on the stairways used to access the belt conveyors on line 4 (stairs welded to the side of the belt conveyor trolley) (2 sets). In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
