29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Handle Mill: On or about March 6, 2018 the employer had not developed and documented energy control procedures for equipment on site including, but not limited to, the L-M Pond and Deck Saw Model 75 HC Serial Number 1765 that outlined the scope, purpose, authorization, rules and techniques to control hazardous energy and the means to enforce compliance with the procedures. This equipment has hydraulics and electric as its energy sources. (b) Handle Mill: On or about March 6, 2018 the employer had not developed and documented energy control procedures for equipment on site including, but not limited to, the Log Carriage that outlined the scope, purpose, authorization, rules and techniques to control hazardous energy and the means to enforce compliance with the procedures. This equipment has electric and air as its energy sources. (c) Handle Mill: On or about March 6, 2018 the employer had not developed and documented energy control procedures for equipment on site including, but not limited to, the Head Saw that outlined the scope, purpose, authorization, rules and techniques to control hazardous energy and the means to enforce compliance with the procedures. This equipment has electric and air as its energy sources. (d) Handle Mill: On or about March 6, 2018 the employer had not developed and documented energy control procedures for equipment on site including, but not limited to, the electric operated Dowel Milling Machine that outlined the scope, purpose, authorization, rules and techniques to control hazardous energy and the means to enforce compliance with the procedures. This operation has had a history of serious injuries related to improper energy isolation.
