29 CFR 1910.146(c)(1): The employer shall evaluate the workplace to determine if any spaces are permit-required confined spaces: a. On or about February 28, 2018, located at the Curing Room and Mandrel Hose "Geese" Curing Rooms, the employer did not evaluate their confined spaces for existing or potential hazards when employees entered the confined space when pushing and pulling the mandrel carts into and out of 16 autoclaves. The autoclaves had limited means of egress and potential for hazardous atmospheres, thereby exposing the employees to contact with steam and/or hot surfaces. b. On or about February 28, 2018, located at the Banbury pit, the employer did not evaluate their confined space for existing or potential hazards when employees entered the pit to perform cleaning and maintenance activities to equipment. The pit had limited means of egress, unguarded rotating parts, potential for unexpected startup and/or the release of stored energy, and fall hazards, thereby exposing the employees to caught-in, stuck-by and fall hazards.
