29 CFR 1910.119(e)(3)(iii) The process hazard analysis did not address engineering and administrative controls applicable to the hazards: a) GEA/FES and Frick ammonia refrigeration systems: The process hazard analysis failed to address how operators working on the roof will be notified of an emergency condition when there was inadequate reception for the wireless phones provided for communication and no alarm system on the roof, on or about 10/10/17. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
