29 CFR 1910.28(b)(5)(i): The employer did not ensure each employee on a runway or similar walkway was protected from falling 4 feet (1.2 m) or more to a lower level by a guardrail system. On or about January 18, 2017, and times prior thereto, in the Grey Production Area, where employees were exposed to fall hazard exceeding 4 feet when filling a cement hopper from a work platform without being protected by a guardrail system.
