29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds per employee attached, or were not designed, installed and used: Location: 325 Kent Avenue, Brooklyn, NY. 4th Floor North of South Tower. a) Employees were working on the exterior façade of the building approximately 20 feet high from a Dunlop 12000 mast climber scaffold. The employer did not determine that the anchorage attachments used were not capable of supporting 5,000 pounds, as part of their fall protection systems. The Professional Engineer's Designed anchorage point was not used by the employer and anchorages were attached to mast sections under the decking. On or about 12/09/2016 and 04/11/2017.
