29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: (a) 312 Old Goff Mountain Road: On or about October 12, 2016 and time prior, the fall protection systems provided by the Company for use by employees were not inspected prior to use and defective equipment, such as snap hooks and rope grabs with visible signs of corrosion and vertical lifelines/ropes with visible deterioration and damage were being used by employees when working at heights up to 15ft. 9in. above the ground.
