29 CFR 1926.1053(b)(1): Portable ladders are used for access to an upper landing surface and the ladder side rails do not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access: The employer did not ensure portable ladders were constructed and placed so three (3) feet or more was extended above the upper landing. Workers performing construction of an electronic sign were exposed to falling approximately eleven (11) feet while using a folding ladder to ascend/descend from the top of the sign to the ground.
