29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Maintenance Shop: On or about June 25, 2015, and times prior, the Round Tower mill with a 20 inch drill press, Serial Number 8407124, used by maintenance employees to drill holes in steel was not provided with an adjustable guard to cover the unused portion of the bit and chuck above the material being worked. (b) Maintenance Shop: On or about June 25, 2015, and times prior, the Central Machinery 20 inch drill press, Serial Number 0504130045, used by maintenance employees to drill holes in steel was not provided with an adjustable guard to cover the unused portion of the bit and chuck above the material being worked. (c) Maintenance Shop: On or about June 25, 2015, and times prior, the American Tool Works Corporation lathe, used by maintenance employees was not provided with a chuck guard to protect the operators from being struck by the 12 inch rotating chuck. (d) Maintenance Shop: On or about June 25, 2015, and times prior, the Central Machinery horizontal/vertical bandsaw, Serial Number 358451504, used by maintenance employees to cut steel and pipe had an 8 inch section of unused blade that was not guarded.
