29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: (a) On or about March 31, 2015, in the fabrication shop, the employer failed to ensure the Miller arc welder, serial number JD727836 fittings were effectively closed. Employees were exposed to electrical hazards. (b) On or about March 31, 2015, in the fabrication shop, the employer failed to ensure the Lincoln Ideal Arc 500, serial number 859837 fittings were effectively closed. Employees were exposed to electrical hazards. (c) On or about March 31, 2015, in the fabrication shop, the employer failed to ensure Panel A transformer fittings were effectively closed.
