29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. The employer is failing to provide adequate powered industrial truck training to employees tasked to forklifts and shag trucks throughout the facility. This was most recently evident during the inspection of the facility located at 2500 Old Hadar Rd & 2300 Riverside Blvd. Norfolk, NE 68701; where the employer has employees operating the shag truck and multiple sit down propane powered forklifts throughout both facilities with having conducted adequate training. Instances include but are not limited to: i. Employees forced to sign certification stating that they had received a hands on evaluation when in fact they had not. ii. Employees took a written quiz where the instructor provided all the answers thus failed to conduct an accurate assessment of the employees training.
