29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: (a) On or about March 4, 2015, an employee was struck in the head with the forks of a powered industrial truck, resulting in a fatal injury. The employee was working below the suspended forks and a compressed gas cylinder and was not using head protection.
